Unified OpenStack and Ceph Monitoring via Prometheus and Grafana

Description

Current the OpenStack and Ceph are divided monitoring, we should unify them to one Prometheus and Grafana with different exporter.

And for ceph cluster grafana dashboard, it needs to install plugin vonage-status-panel and grafana-piechart-panel.

Currently, kolla-ansible support external ceph ckuster monitor integration.

One lacking piece is the new ceph-iscsi monitor integration which we need to add to kolla-ansible.
